Description
We are currently seeking qualified candidates for a full time Long Term Care Nurse Practitioner (NP) position in the Richmond area.
Job Description:
The NP provides general medical care and treatment to patients in medical facility such as clinic, health center, or public health agency, under direction of Physician, by performing the following duties. Responsible for in-house nursing facility visits to hospice patients for the purpose of conducting the face-to-face 60 day re-certification encounter.
Qualifications:
- Licensed Nurse Practitioner in the State of Texas
- 1+ years of experience as a Nurse Practitioner
- Experience working in Long Term Care settings (NF, ALF, Memory Care, Personal Care Homes) - preferred
- Experience with Hospice, Home Health, Palliative Care, and/or Geriatrics - preferred
- Effective written and verbal communication
- Ability to travel - must have reliable transportation
- Clean background, drug screen, and driving record
